When you fill in an on-line holiday application please remember that:
* The people who work in the airport don’t know you
* If your passport doesn’t match your flight ticket, you ain’t going anywhere
* If the on-line form says “name, as shown on your passport” you should give your name as it is shown on your passport
* Don’t complain when you have to buy another flight ticket (if you still want to go on holiday)
So:
Jim, Marge, Betty, Bill, Jack, Flo, Paddy, Stan, Maggie, etc, etc, please take note, "if" this is how your name is shown on your passport, you're OK, if not you "will" have a problem..

Mr jocrad is known by his middle name but obviously passport, driving licence etc all in his "proper" name - even after 16 years I still have to stop and think before I book anything!!

I'm familiar with this as with nearly everyone, apart from my mother and siblings, I use a name completely different from the one on my passport, so have to ensure they are aware of the correct name if they are booking anything for me where the name needs to be my 'official' one. Another situation where I found it to be a problem was when I was given a cheque to cover a work related expense. Although the person writing the cheque deals with all staff records etc. and therefore knows the name I am employed under, it obviously slipped her mind when she wrote the cheque.

Iski, Why not just use your 'assumed' name for everything? My mother has not used the name which is on her birth certificate since she started secondary school (around 1943) - all official documentation shows the name she uses (including passport, driving licence, bank accounts). Her (Irish) marriage cert shows her assumed name with her birth name in brackets; my (English) birth certificate shows her assumed forename and not her given name.

Yellow Pyranha, Originally I didn't change it on official documents because I knew it would upset my Dad (who chose my name). It now doesn't seem worth the bother of changing all the documentation as it only occasionally causes a problem. That's interesting about your mother, though.
