@NickyJ, Not NHS, private one. NHS physios just did assessment and MRI referral. Also kicked off with Pilates. My gym has only reformer pilates classes but I do fundamental one, that one is slow and easy

Do you have somebody checking your movements? It is so very easy to let other muscles dominate without realising it.

Do you have somebody checking your movements? It is so very easy to let other muscles dominate without realising it. |
We are 6 people in a class only and have PT but it is more gym class than a physio..

Good hopefully if you find them regularly stopping you and making you do it again / slower / smaller movements that would be a VERY good sign

@Markisa, my experience for what it's worth. If you're in anyway worried, you've had MRI scans, pay to get them on USB then go to see Mrs Bell. I had what I thought was a rather good NHS physio but the exercises he had me doing were actually not good for my knee. A trip to Wimbledon and following their advice and physio I don't limp anymore (normally!) Ok, so my knees still swell up like a couple of watermelons after a week's skiing but at least I can do a week now. Serves me right for riding, and falling off motorbikes for the last 41 years I suppose.
Sometimes the NHS can be great, other times you just have to go to a private specialist.
(I'd still like to try this newfangled regrowth of cartilage though.)

@Markisa, Now you have MRI results which show very little then the physios will be able to be a bit more bold with what they offer you. Suggest as @NickyJ, says maybe worth getting a private physio to see you once or twice a week for a couple of weeks to see if you can turn it round. The main advantage of private is you pick your physio, your time and frequency.

@Jonathan Bell, i ve been seeing a private physio for December once per week, he sent me to MRI and also wasn't sure if I need to continue with physio as no progress, so we stopped. After MRI he advised me to go to gym, sauna and continue with physio exercises to see if it helps and also start with naproxen to see if it helps to bring swelling down.
I had another appointment with FCP who referred me now for NHS physio. Though it may take a month or two for me to see them. In the meantime I am planning to go to another private physio to see if it works better with another person. I have holidays upcoming this week for next two weeks but once I am back I will try another private physio for couple of months before NHS one comes through.
Knowing that I have no damage, I manage to push myself a bit harder with physio exercises and biking/swimming/stretching/foam rolling at gym. This seems to be working. My ROM improved to maybe 140 (other leg 155). Still have stiffness especially in the mornings and when leg is one position for too long. Almost no limping now but still some swelling at front where I landed and soreness especially in lower hamstring area.
Still not sure if skiing will be feasible in 8 weeks mainly because of pressure of ski boots on shin bone which is still a bit hurtful when i strongly press on it, not surprising though given how large the bruise was.... Was thinking to try snowboarding instead as I will be in UCPA and they can swap a course, but i ve heard lots of fall on knees when learning so this will be probably off. But it is only 2 skiing days aka extended weekend but in worst case scenario I will just spend a weekend by Geneva lake instead of Chamonix.

@Markisa, wondering if you have scar tissue build up?
In rehab for my ACL recon that was causing me limitations in getting back full ROM. The physio gave me a good sports massage (OMG it was painful) but helped to break it down some. I can't saying getting a sports massage won't hurt (as it will!) However it won't cause further damage

I was thinking about it as well. It is either scar tissue or swelling which prevents full ROM. I am working on it by using foam rollers and gun massager which i have in a gym and do stretches. The also have sport/deep tissue massage in my gym and I also was planning to check with physio if I need one.
I am leaving this friday for holidays in the sun, upon return will go to another private physio and massage if necessary. Would ve never expected such a long recovery after simple fall on a pavement but oh well. could be worse i guess

I find them beneficial as part of rehab.
I have similar thoughts wrt to my elbow, I bashed it on the wall trying fit past my daughter sat at the table about 6/7 months ago. I am currently sat in waiting room to see consultant over it!

Oh! No idea what it is but hopefully nothing too serious. I am still shocked how long symptoms from such simple things such at banging your elbow against the wall or straighforward fall on pavement can last. Would have never thought so.
Do you recall how long /how many massage sessions did it took to bring down scar tissue? I am not sure if my ROM restricted by scar tissue or still by inflammation as I have relative big lump on medial side of my knee just below the knee cap. Could be either or both of them.

Only a few, a lot of the good from first one, and they sent me off with instructions on how to work on it in between. Might have just been 2 actually. Definitely recommend 1, and see if it feels like it makes any difference

@Markisa, if you suspect inflammation (I would too, though there's usually a root cause to swelling which may be undiagnosed) and you're going away relaxing for a bit, can you not get a steroid injection from your GP before you go to see if it settles with the rest whilst you're away? Especially if you're flying because the air pressure change on a flight won't do your swelling any favours.

@Je suis un Skieur, FCP said there is no need as risks outweight benefits in my case and they want to continue with physiotherapy.
Do you have an idea what could be a root cause to swelling if MRI and XRAY are primarly clean?

Thank you NickyJ, definitely will try that once I am back from hols.
In the meantime I am getting slowly better, waking up with less stiff week this week than previous one though still far from normal. Will continue doing physio exercise till I back and taking naproxen hopefully will help bring inflammation down and improve symptoms.

@Markisa, as I said in my previous post, I've had a torn meniscus that didn't show up on MRI and I've had hairline fractures/bone contusions that didn't show up on X-ray but were obviously there. I believe an MRI should show a hairline fracture but a CT scan is better? Both my right humerus below the elbow and one of my knuckles still have lumps on the bone 30 years after X-rays said there was no injury but that was before MRIs and CT scans.
I've had a bone contusion that was diagnosed with an ultrasound scan, I believe the window for an MRI to detect a bone bruise is quite brief. Your ongoing issues certainly seem similar to the injury I had to my elbow, which was also caused by landing on a hard surface.

Agree Cary can fail to “ see”
a hairline fracture. If there is a fracture present MRI will flag its presence up better than any other modality. CT is very good but MRI will pick up the transition from a bone bruise through to fracture. If you want to be able to “ see “ a fracture line CT may have some benefits over MRI. But as a screening tool MRI is the best. Bone bruising frequently persists for 12 weeks after something like an ACL rupture. In fact the pattern of bruising can almost on its own be diagnostic. MRI is very good for meniscal tears but I see a few a year where the MRI has not been able to “ see “ it. MRI is still the modality of choice for diagnosing meniscal tears Jonathan Bell

@Jonathan Bell, what could be other reason of knee soreness/stiffness from a fall for 3+ months if no ligament damage, no meniscus damage, no fracture and not even bone bruising detected? It just surprises me, the healing time..

Sounds to me like the diagnosis is being missed.There are some places I won’t use for cheaper scans. They do scant views and some of the radiology reporting is dreadful. PCL injuries are difficult to diagnose if you don’t specifically ask the radiologist to comment. Falls into front of the knee can be slow to settle as they frequently upset the patella joint and fat pad. PCL injuries are relatively uncommon but are frequently missed on scans and they can lead to stiffness and pain in the knee but all of these would be amenable to physiotherapy. what part of the country are you in? I may be able to recommend a physiotherapist. Jonathan Bell

@Jonathan Bell, thank you. I am in north London, Stanmore/Edgware area. Just back from holidays, stiffness is now fully gone, but still have some soreness and cannot yet sit on my heels. Thats the only thing that remaining atm from all my symptoms. So still keen to do a physio for couple of weeks. If you can recommend anyone, would be great.
Not sure in regards to PSL, I had 3 examinations performed by different physios, they certainly did ACL tests, not sure for PSL, but I have soreness mainly in front of the knee exactly where I landed upon fall. NHS sent me to do MRI in Inhealth center in Ealing. Not sure if they are cheap/good or not. I though PSL is the thickest ligament in the knee so full tear would be noticeably on MRI. The only thing their MRI found is subtle chondral fissuring with subchondral bone marrow oedema in front around knee cap and bone below and tiny baker cyst in the back. I could feel the cyst in January when straightening the leg, but not any longer. Now it is just a bit of soreness around knee cap and on bone below it in front.
