Why not try a three piece style boot? Some touring boots (Scarpa Maestrale range for one) haev a tongue that folds across from the side rather than the normal overlap. I think Full Tilts are the same?

@meep, yes the tongue can be flattened out with heat so it digs in less. But it sounds as if you would be far better off in a wrap type liner such as an Intuition or Palau. Really worth getting the advice of a decent bootfitter.

as others have said an intuition or palau wrap style liner is going to be the most appropriate solution for you, this is custom moudled to your foot / leg but as it is a spiral wrap there is no tongue as such, the wrap can be allowed to adjust to an extent as t=well so it can cope with the fluctuations in calf size, the other thing which may help (with the guidance of your doctor!) Is a compression sock

I think that you should go to a specialist boot fitter you can get a special made liner with extra depth to accommodate a larger calf and also a low wide cuff. But you will have to accept that decathlon will not have these products. However if you make the correct purchase first time round you will only end up purchasing one pair of boots which in the long term will end up more cost effective.
